We enclose a permit blank which must be signed before we can place the headstone in the lot at Mount Auburn

If the lot has perpetual care the [?] [one?] should be signed but if not then the white [one?] Then please [predate?] in the enclosed envelope & mail it

There seems to be a great deal of ceremony which these matters [?] the signature [?] and the authorities treat us all alike in these matters
