Not sure if this is the correct name for this certificate, but the name of the child does not match the name I found on Find a Grave's site. The name on Find a Grave (Jessie Lee Wallen) matches the date of birth (March 27, 1923) and the sex of child (on this form, 'Boy' is crossed out and replaced with 'Female'). I went ahead and filled it out as it's written on the certificate.
